Allergy
Associates of Utah now has two conveniently located facilities to serve you.
Our original Cottonwood office is central to both I-15 and I-215, across the
street from Fashion Place Mall. Our Jordan Landing office brings ease and
accessibility to the fast growing SW end of the Salt Lake
Valley.

Our routine
office hours are Monday, Wednesday, and Friday from 8 a.m. to 5 p.m. and Tuesday
and Thursday from 10 a.m. to 7 p.m. Also, we are open Saturdays at our
Fashion Place location from 8:00 am - 2:00 pm. Injections are given no later than
4:45 p.m. M-W-F and 6:45 p.m. T-TH, so that you will be able to wait 20 minutes
in the office following your injection.

For your convenience, Allergy
Associates of Utah offers an online form to fill out out prior to your
initial visit. Click on the green box below. This
should open a PDF form that you can print off and fill out prior
to coming in the office.
The
initial
visit to Dr. Rogers will involve a detailed history
in an effort to define the factors causing your allergy or
triggering your asthma. A physical examination follows.

Armed with this information, Dr. Rogers is able to determine
what allergy tests (skin & sometimes blood tests) are needed to
diagnose and confirm the severity of your allergy.
Breathing, or pulmonary function tests, are used to evaluate
your asthma.
A treatment
program can now be designed and tailored to your individual needs. The
program may include environmental control, medication or allergy injections, as
well as specific patient and family education relating to you.
Telephone Policy
Please
confine routine calls to office hours. We also request that you have your
pharmacy call us directly during office hours for prescription refills.
After hours, on weekends and holidays, your call will be taken by our answering
service to be forwarded to Dr. Rogers. On rare occasions, another qualified
physician will be taking his calls. If your call is not returned within 30
minutes after reaching the answering service, please let us know.

Injection Policy
Although
allergy serum therapy may be quite helpful for controlling your allergies, there
are inherent dangers. In spite of all precautions, significant reactions still
rarely occur. Most significant reactions occur on an immediate basis; therefore,
you are asked to remain in the doctor's office for at least 20 minutes following
an injection.
Fees, Billing and
Insurance
Fees are due
at the time of services rendered unless other arrangements are made. If you
anticipate a financial problem, we will help you work out an acceptable payment
plan. If you are ever dissatisfied about a charge or feel that an error has
been made on your account, our business office will be happy to discuss it with
you.
Our staff
will attempt to inform you of how much your insurance company will pay for a
service, however, we are not responsible for this information. Your
insurance benefits may not completely cover our services, and you may be
responsible for the difference. If you need assistance in completing your
insurance forms, we will be happy to answer your questions.
