For your convenience, Allergy Associates of Utah offers an online form to fill out out prior to your initial visit.  Click on the green box below.  This should open a PDF form that you can print off and fill out prior to coming in the office.  The file size is about 1.8 MB, so this may take a minute to open.  You may fill in the forms and check the appropriate boxes and then print it out if this is convenient for you.

NEW PATIENT FORM

The initial visit to Dr. Rogers will involve a detailed history in an effort to define the factors causing your allergy or triggering your asthma.  A physical examination follows. 

Armed with this information, Dr. Rogers is able to determine what allergy tests (skin & sometimes blood tests) are needed to diagnose and confirm the severity of your allergy.  Breathing, or pulmonary function tests, are used to evaluate your asthma. 

A treatment program can now be designed and tailored to your individual needs.  The program may include environmental control, medication or allergy injections, as well as specific patient and family education relating to you.
