Facility:
The South
Valley Dermatology Center is conveniently located at 3570 West 9000 South, Suite
220 West Jordan, UT. This is on the campus of the Jordan Valley Hospital.
This is the only dermatology clinic south of 3500 S. and West of State
Street. Unlike years ago when
patients would have to drive downtown to see a specialist, the specialist is now
right in your neighborhood!:
Insurance:
We accept most
insurance including local plans, Medicare, and Medicaid.
Unfortunately, we are not on
the IHC Select Med or AFC panels.
We are proud to go the extra mile where insurance claims are concerned by
offering to submit claims to both primary and secondary insurance carriers.
We cover the cost of staff time, forms, and postage, thus making this
service free of charge to our patients.
All we need from you is accurate insurance information, which you can
leave with us as you register before your first appointment.
In order to be able to continue to offer this service, we request that
co-payments and deductibles be paid at the time of service unless other
arrangements have been made.
Scheduling:
Generally, we
can schedule routine patients for an appointment within one week of receiving a
telephone inquiry. We try to leave
a couple of slots open each day and thus can often offer same day service in
case of emergency. A brief
explanation of the patients’ problem to the receptionist will allow her to
schedule the appropriate amount of time for your appointment.
We know that
your time is valuable and we make every effort to be on time for appointments.
Rarely, there may be patient emergencies in the office or at the
hospital, which can cause schedule delays.
We will always let you know as soon as we become aware of a delay. At that time, you may continue to wait or reschedule your
appointment. We appreciate your
cooperation and will work with you in any way we can.

Occasionally, a
patient may be seen before you even though they arrived after you.
This would most likely be for a brief visit with the nurse or our nurse
practitioner and should not delay your visit.
In order to
maintain an optimal schedule, we request that you do not bring family members to
be seen at the same time if you have scheduled only one appointment.
Of course you may schedule family members in back to back time slots. Please call before coming in if someone needs a work-inc
appointment for urgent care.
Finally, as we
are unable to guarantee the availability of a staff person to look after an
unattended child, please arrange for childcare.
Telephone:
To be fair to
scheduled patients, we make every effort not to interrupt our physician during a
patient visit. Your call will be taken by one of our well-trained staff
members. Please answer any question
they have for you. If necessary,
arrangements will be made for Dr. Forsha to return your call at a specified
time. Working messages through our
receptionist will allow us to serve your needs as quickly as possible.
Most medical
problems are not treatable by telephone, and generally require an office visit
for proper review and diagnosis.
Prescription refills by phone are approved during office hours only when we have
access to your medical record and only when you have been seen within the last
year.
